Additional Summer Camp 2024 Child Forms

St. Luke’s Day School requires additional forms for each child who is new to St. Luke’s Day School for Summer Camp 2024. These forms are separate from your child’s application which has already been submitted. The deadline for submission of completed forms is May 17, 2024 regardless of summer camp start date.  If your child attended SLDS during the 2023-24 school year, we will use the school year forms already on file.

For each child enrolled:

1. Download (or print), complete, and save the following – all are fillable pdfs except the emergency cards

   2. Authorization to Pick-Up 2024
   3. Emergency Card 2024 (scroll down for instructions)
   4. Introduce Us to Your Child 2024
   5. Parent Agreement 2024
   6. Discipline and Guidance 2024
2. Obtain your child’s most recent Immunization Record
3. Upload all forms and the immunization record HERE

You will receive a confirmation email upon successful submission of forms. If you have any trouble with the upload, you may email forms directly to Sarah Scott at sscott@stlukesmethodist.org.

Medical form instructions:

• The date of your child’s last well check and pediatrician’s signature on the top portion medical form are required by Licensing. (Doctor’s signature on the shot record does not meet requirements.)
• Health insurance information and parent signature on the bottom portion is required by Licensing.
• Do not delay! Many pediatricians require a week or more to complete a medical form.

Emergency Card instructions:

• • Complete top and bottom of both pages, even though information is repeated. One card goes to classroom, and one is filed in the office.
  • A local emergency contact is required by Licensing.
  • Parent signature is required.
