2019-2020 Child Enrollment Forms
St. Luke’s Day School requires the following forms for each child’s 2019-2020 enrollment. For your information, the 2019-2020 Annual Calendar is also available below.
Return completed forms together in a 9 x 12 envelope with your child’s name in the upper right hand corner. Due date: July 1, 2019.
Complete and print all six (6) fillable pdf forms and note any special instructions. One set of forms for each child; two copies needed where indicated. All blanks on all forms must be completed and parents must sign and date each form.
Completed forms are due in the Day School office no later than Monday, July 1. A $50 penalty will be assessed for late or incomplete forms.

Medical Form – 2 copies required – Doctor’s signature, date of last well check*, immunization record**, insurance information and parent’s signature are required.
Medical form is due by July 1 regardless of child’s birthday or scheduled appointments. For well checks scheduled after July 1, please see below.

Parent Agreement Form– 1 copy required
Discipline and Guidance Policy– 1 copy required

Medical Form Notes:
*Texas Health and Human Services requires that children receive regular well checks on the following schedule: 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, and annually age 2 and up.
**Children must be up-to-date with immunizations to attend St. Luke’s Day School.
For well checks scheduled after July 1, 2019:
Texas Health and Human Services and the Houston Health Department require St. Luke’s Day School to maintain up-to-date health records on every student. For every well check that takes place after July 1, 2019 and throughout the school year, please have your pediatrician complete the Medical Information Update and return it to the Day School office within one week. Include the most recent immunization record. This will be added to the child’s original medical form.
