Child Enrollment Forms
St. Luke’s Day School requires additional forms for each child who has a confirmed place for the upcoming school year. These forms are separate from your child’s application which has already been submitted. Completed forms are due no later than July 1, 2024. A $50 late fee will be assessed on 7/2 for late or incomplete forms. An additional $50 will be assessed on 8/2 if forms are still late or incomplete. Please note: Returning students (enrolled for 23-24 school year) need Online Enrollment Form and Family Handbook Acknowledgement ONLY if medical records are up to date (see details below).
FOR NEW STUDENTS for the 24-25 school year, parents will fill out and submit ALL FORMS. There are five steps to this process. For each child enrolled:
- Complete and submit Online Enrollment Form
- Review the 2024-25 Family Handbook and complete the 24-25 Family Handbook Acknowledgement HERE.
- Download and complete
- Medical Form 24-25 (scroll down for instructions)
- Discipline Guidance Form (new students only)
- Obtain your child’s most recent Immunization Record
- Upload Medical Form, Immunization Record, and Discipline & Guidance Policy HERE
You will receive a confirmation email upon completion of Step 1 (online form), Step 2 (acknowledgement), and Step 5 (uploads). All steps are required for your child’s enrollment forms to be complete.
FOR RETURNING STUDENTS (enrolled for 23-24 school year), parents will complete and submit Online Enrollment Form and 24-25 Family Handbook Acknowledgement. Medical records (medical form and immunization record) must be submitted only if school records are out of date.
- Complete and submit Online Enrollment Form (updated annually)
- Review the 2024-25 Family Handbook and complete the 24-25 Family Handbook Acknowledgement HERE.
- Determine if your child’s medical records are up to date. Any child with a well check due in May or June will need a new medical form/immunization record. All others may contact Kate Trepa (ktrepa@stlukesdayschool.org) to check on the status of their child’s medical records. If records are up to date, you are done. If records are not up to date, move to step 4.
- If needed, download and complete
- Medical Form 24-25 (see details below)
- If needed, obtain your child’s most recent Immunization Record
- Upload Medical Form and Immunization Record HERE
You will receive a confirmation email upon completion of Step 1 (online form), Step 2 (acknowledgement), and Step 6 (uploads if needed).
Please note: Updated medical forms and immunization records must be submitted for all well checks/shots after July 1 and throughout the school year.

Medical form instructions:
- The date of your child’s last well check and pediatrician’s signature on the medical form are required by Licensing. (Doctor’s signature on the shot record does not meet requirements.)
- Medical form is due by July 1 regardless of child’s birthday or scheduled appointments. For well checks scheduled after July 1, you will be required to submit an updated form prior to the start of school.
Additional information:
- Children must have proper documentation of required immunizations or official affidavits of exemption to attend St. Luke’s Day School. The schedule of required immunizations for the state of Texas may be found HERE
- Texas Health and Human Services (Licensing) and the Houston Health Department require St. Luke’s Day School to maintain up to date health records on every student throughout the school year. Parents are required to submit an updated medical form and immunization record following every well check their child receives after July 1, 2024. Please note that a doctor’s signature on the immunization record does not meet Licensing requirements.
- Texas Health and Human Services requires that children receive regular well checks on the following schedule: 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, and annually age 2 and up.
