St. Luke’s Day School Volunteer Background Checks

We have many volunteer opportunities at St. Luke’s Day School! In order to volunteer with children in the classroom, you must complete a Volunteer Background Check. Please complete one of the forms linked below. Once complete and approved, you will be contacted. All Background Checks are valid for one year and must be renewed annually.

Initial Volunteer Background Check Form
Please complete this form if it is your first time doing a background check at SLDS.

Renewal Volunteer Background Check Form
Please complete your renewal for the current year, only if your initial application was through St. Luke’s Day School.

SLUMC Safe Sanctuary Preapproval
Please complete this form if you are currently Safe Sanctuary approved through St. Luke’s United Methodist Church.
