REGISTRATION POLICIES
Luke’s Place Full-day Program
2022-2023
Definition of Acceptance
As stated in the enclosed letter, your child has been accepted by St. Luke’s Day School and has a place reserved for the 2022-2023 school year. We expect that your child will attend SLDS for the entire 2022-2023 school year (September 2022-August 2023) and the following policies now take effect. Important deadlines for payments and forms completion must be met or your child’s place will be at risk.
Fee Payments and Forms Deadlines
To confirm enrollment and hold your child’s place, payments and forms for the 2022-23 school year are due on the following schedule. ACH bank drafts and credit card charges will automatically occur on the specified dates. If payment method is check, it is due by that date.
March 14-18, 2022 Registration fee will be charged and checks will be deposited. The registration fee is non-refundable.
April 14, 2022 September 2022 tuition deposit will be charged and is non-refundable once paid. Those wishing to decline enrollment and cancel this payment must do so by Friday, April 1 (see below).
May 1, 2022 Child Enrollment Forms for 2022-2023 school year will be available online. This includes a medical form which requires a doctor’s signature to be complete. Child Enrollment Forms must be updated annually, no exceptions.
July 1, 2022 Completed 2022-2023 Child Enrollment Forms are due (submitted online). A $50 fee will be assessed for late or incomplete forms.
August 2, 2022 August 2023 tuition deposit will be charged, and is non-refundable once paid.
August 2, 2022 Final deadline for late 2022-2023 forms.
September 1, 2022 2022-2023 Activity/Supply Fee will be charged and is non-refundable once paid.
Start of school Emergency Information Cards will be distributed and are due your child’s first
week of school.
To Decline Enrollment
Parents have until Friday, April 1, 2022 at 5:30pm, to decline enrollment in writing. Email is acceptable (Tessa Waltz: twaltz@stlukesmethodist.org). Please include the reason for declining and/or the school your child will attend; that information is very helpful to us. Once enrollment is cancelled, no further payments will occur. The registration fee is not refundable.
Withdrawals between April 1 and August 31, 2022
To withdraw a child, parents should inform the office in writing a minimum of 30 days before the date of withdrawal or earlier if possible. The school needs time to fill any openings so we may continue to meet our expenses. The September 2022 tuition deposit will be forfeited. See the 2022-2023 Family Handbook for withdrawal policy after the start of school; 30 day minimum notification remains in effect.
Immunizations
As required by the Texas Department of State Health Services, all students must be up-to-date on required immunizations to be enrolled in St. Luke’s Day School. Due to the age of the children we serve, we do not accept exemptions. Visit https://dshs.texas.gov/immunize for the immunization schedule.
Tuition Payments
St. Luke’s Day School is intended to be a twelve-month commitment, and each payment represents one-twelfth of the school year. Credit is not given for a child’s absence or days the school is not in session.
Deposit Transfers
Tuition and fee deposits paid are not transferable to another child in the same family if one child withdraws. For example, if a family registers two children, and one withdraws to attend another school, the registration fee and any non-refunded tuition for the withdrawn child are forfeited. This amount will not be transferred toward tuition due for the remaining child.
Important School Start-up Dates
The 2022-23 School Year calendar, including meet the teacher and parent orientation dates, as well as the first day of school, will be published on our website May 1, 2022.
Parents will be informed of classroom and teacher assignment, location and time to meet the teachers, and additional school year details in mid to late August.
Up-to-date Children’s Records Required
It is very important for you to keep the Day School office updated on any changes in your contact information, including phone numbers, and home and email addresses.
Texas Health and Human Services (a.k.a. Licensing) and the Texas Department of State Health Services require SLDS to maintain current health records for every child, including well-check documentation and up-to-date immunizations. The state licensing agency also requires that children have regular well-checks with their pediatrician. Each time your child has a well-check, you will need to send two items to the Day School office:
- a completed Medical Information Update form signed by your child’s pediatrician,
- a copy of your child’s updated immunization record.
Medical Update forms are available on our website: www.stlukesdayschool.org.
For Lambs Class Only: Start Date for New Babies
The first day of school will be in early September. New babies in the Lambs class must be 3 months old in order to attend. If your maternity leave is less than 3 months and you need care earlier, please contact Tessa Waltz to discuss your specific situation. The September 2022 tuition deposit will be applied to September; the school incurs all expenses associated with the enrollment of your child whether or not he is attending.
