ANNUAL ENROLLMENT FORMS – DUE JULY 1, 2026
St. Luke’s Day School requires additional forms for each child who has confirmed their placement for the upcoming school year.
These forms are separate from your child’s application which has already been submitted. Your child will not be able to attend school until all forms are complete and up to date.

2026-2027 PAYMENT SCHEDULE
Parents have a designated form of payment on file that will be automatically charged based on the published schedule.
May 4, 2026 – May 2027 tuition deposit charged for SLDS Part-Day
August 5, 2026 – August 2027 tuition deposit charged for Luke’s Place Full-Day
September 3, 2026 – Activity/Supply Fee charged for all programs
Monthly tuition will be charged on the first of each month, October through April for SLDS Part-Day & Kaleidoscope TK/TK FD; October through July for Luke’s Place Full-Day.
