Application Submission

Before you submit your application, please make sure you have done the following:

• Check your application to make sure you have completed all sections.
• Save the document to your computer using your child’s name.
• Upload one form per child (or application).

Once submitted, you will receive a confirmation email. If you do not receive the confirmation (check your spam folder first), or have questions, please email Sarah Scott (sscott@stlukesmethodist.org).
