Enrollment Forms Deadline: July 1, 2026
A $50 late fee will be assessed on July 2 for late or incomplete forms. Contact Sarah Scott with any questions.
Required Annual Forms for New Students:
- Online Enrollment Form
- Family Handbook Acknowledgement
- Download and complete the Medical Form *must be signed by your pediatrician
- Obtain your child’s most recent Immunization Record
- Download and complete the Discipline & Guidance Policy
Once complete, upload the Medical Form, Immunization Records, and Discipline & Guidance Policy using the button below.

- Obtain your child’s most recent Immunization Record
Once complete, upload the Medical Form, Immunization Records, and Discipline & Guidance Policy using the button below.
*Please note: If your child’s medical records are currently up to date in our system, you do not need to submit a new medical form and/or immunization records until the next well-check date. If you are unsure of your child’s status, please reach out to the School Nurse, Kate Trepa.
Additional information:
- Children must have proper documentation of required immunizations or an official affidavit of exemption to attend St. Luke’s Day School. The schedule of required immunizations for the state of Texas may be found HERE.
- The State of Texas Department of Family and Protective Services require St. Luke’s Day School to maintain up to date health records on every student throughout the school year. Parents are required to submit an updated medical form and immunization record following every well check their child receives. Please note that a doctor’s signature on the immunization record does not meet Licensing requirements.
