Annual Trainings
Child Abuse training must be completed annually. At the conclusion of training, please complete the form below.
The SIDS/Shaken Baby training should be completed annually. At the conclusion of the video, please complete the form below.
The Health Procedures training should be completed annually. Please read each section carefully before completing the required quiz below. A passing score is required to complete this training.
Communicable Diseases & Prevention
Immunizations
- All children must be up to date on immunizations required by Texas DSHS or submit an official affidavit of exemption.
- Immunization records are managed by the School Nurse.
Preventing the Spread of Illness
- Wash adults’ and children’s hands with soap and running water for 20 seconds:
- After toileting or diaper changes
- After outdoor play
- Before eating
- After coughing, sneezing, or blowing noses
- Any time hands are visibly soiled
- Cover coughs and sneezes with a tissue or elbow.
- Clean and sanitize rest mats, cribs, mouthed toys, pillows, dramatic play clothing, and classroom surfaces according to the NAEYC frequency table in the Staff Handbook.
- Report any diagnosed illness to the School Nurse, who will notify classroom families.
- Children who become ill during the school day should be taken to the School Nurse.
- If the child is too ill or too young to walk, radio the nurse for assistance.
Food Allergy Awareness & Response
- Food allergies are managed by the School Nurse.
- Children with food allergies or sensitivities are listed on the orange fold-over card in the classroom.
- Parents of classrooms with food restrictions are notified by email.
- Children with diagnosed food allergies have:
- An Allergy Action Plan (AAP) from their physician
- Rescue medications stored in the AUX Office
Recognizing & Responding to Allergic Reactions
- If you suspect an allergic reaction:
- Take the child to the School Nurse or radio for assistance.
- The nurse will assess and administer medication if needed.
- If the nurse is unavailable and you are responsible:
- Follow the child’s Allergy Action Plan.
- Administer oral medication for mild reactions.
- Use an EpiPen for severe reactions.
- In severe cases:
- Call 911 immediately and follow their instructions.
- After calling 911, radio an Administrator and notify the child’s parent.
- Follow the medication instructions on the prescription label and AAP.
Administering Medications at School
- All medications are stored in the AUX Office.
- Parents requesting medication administration must:
- Complete a medication dosing form
- Provide medication in original packaging (prescription or OTC)
- The School Nurse administers medications unless special arrangements are made.
If a Teacher Administers Medication
- Carefully follow the “Five Rights”:
- Right child
- Right medication
- Right dosage
- Right dispensing format
- Right time
- Medication must be administered with a witness present (another SLDS staff member).
- Both the administering staff member and the witness must sign the dosage record.
Responding to Injuries
Minor Injuries
- Minor injuries (bruises, scrapes, small cuts) may be treated by classroom teachers.
- Ice packs are located in:
- Children’s Kitchen refrigerators
- 2nd Floor Teacher’s Lounge
- Always wear gloves when handling blood or bodily fluids.
- Clean wounds with soap and water, apply antibiotic ointment and a bandage.
- Dispose of gloves and supplies in a blue plastic bag in a lidded trash can.
- Complete an Accident Report and review it in person with parents at pick-up.
- Injuries above the shoulders require the nurse to call parents.
Severe Injuries
- Severe injuries (deep cuts, head injuries, severe bleeding, insect stings) must be handled by the School Nurse.
- The nurse will:
- Evaluate and treat the injury
- Notify parents with the teacher present
- Determine if pickup or emergency services are needed
- Make a 24-hour follow-up call if a child is sent home
**IMPORTANT **
- For all injuries, whether minor or severe, Teachers should complete an Accident Report to be shared with parents.
- Teachers should not call parents about injury to a child. The School nurse will make the phone call with teacher present.
- Teachers should not photograph an injured child or send a photo of an injured child to parents.

– Central Plant – foyer to maintenance and girl’s locker room
– Activity Center foyer
– Sanctuary Acolyte room
– Rotunda between the restrooms
– Johnson Building 2nd floor between the restrooms
– Modern Worship Building in the hallway to the restrooms
– Main Church Office outside the choir room
